New resource alert!
We have developed an information sheet for patients aged 14-24 years who have received a health assessment as part of RAd Health. The information sheet can be given to your patient after a heath assessment, it includes:
- A summary of a young person’s health assessment
- A description of aspects of health covered in the assessment
- A link to our (newly revamped!) patient resources page that can provide your patients with further information and resources should they need it
- What the patient should do next
- A large notes section for you to write any follow up suggestions/instructions for the patient.
Follow this link to download a PDF version of this fantastic new resource.
If you would like the information sheet in another format, or would like us to send some printed copies, please respond to this email.
